CONSENT & WAVIER

1. 1. I, the undersigned parent/guardian, hereby give permission for my child to participate in rugby league training provided by Elite League Performance.
   2. I understand that rugby league is a contact sport and carries inherent risks, including but not limited to, physical injury.
   3. I confirm that my child is physically fit and able to participate in rugby league activities. I will inform the coaches of any health issues that may affect my child’s participation.
   4. In the event of an emergency, I authorise the staff of Elite League Performance to arrange medical treatment for my child, including transportation to a hospital, if necessary.
   5. I agree to hold harmless and indemnify Elite League Performance, its staff, and agents from any and all claims, liabilities, or expenses arising from my child’s participation in rugby league training.

LIABILITY WAVIER

Elite League Performance will take all reasonable precautions to ensure the safety and well-being of all participants. However, by signing below, I acknowledge and agree to the following:

1. I fully understand that participation in rugby league training involves inherent risks and dangers, including but not limited to, serious bodily injury, permanent disability, paralysis, and death.
2. I, on behalf of myself and my child, hereby release, waive, discharge, and covenant not to sue Elite League Performance, its owners, directors, officers, employees, agents, volunteers, and other participants, from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child, whether caused by the negligence of the releases or otherwise, while participating in rugby league training or in any activities related to the training.

I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of Australia, and agree that if any portion is held invalid, the remainder will continue in full legal force and effect.
